    An unusual case of intestinal obstruction by volvulus of Meckel’s diverticulum in a 10-year-old child: a case report

    Intestinal obstruction, gastrointestinal haemorrhage, adherence, intussusception, acute diverticulitis and rarely malignity are the most common complications of Meckel’s diverticulum in children. Reported mechanism of intestinal obstruction in Meckel’s diverticulum include invagination, adherence and volvulus. Obstructive ileum caused by volvulus of Meckel’s diverticilum is not a very frequent medical condition. Here we describe a case of intestinal obstruction caused by volvulus of non-inflamed Meckel’s diverticulum in a 10-year-old child. Considering the potentially life-threatening obstructive ileum in cases of children with deteriorating clinical picture, surgical exploration should not be delayed

    AbstractThe usage of magnetic spherical nanoparticles, coated with substances and driven to targeted areas in tanks, is proposed for cleaning the water from heavy metals. In the present paper, a computational study for the estimation of the optimum gradient magnetic field is presented in order to ensure the optimum driving of the particles into the targeted area. The optimization of the gradient magnetic field rates’ is verified with the particles’ deviation from a desired trajectory. Using the above mentioned method, it was depicted that with the increase of the optimization parameters number, the particles’ deviation from the desired trajectory is decreased

    In recent years, serious energy games (SEGs) garnered increasing attention as an innovative and effective approach to tackling energy-related challenges. This review delves into the multifaceted landscape of SEG, specifically focusing on their wide-ranging applications in various contexts. The study investigates potential enhancements in user engagement achieved through integrating social connections, personalization, and data integration. Among the main challenges identified, previous studies overlooked the full potential of serious games in addressing emerging needs in energy systems, opting for oversimplified approaches. Further, these studies exhibit limited scalability and constrained generalizability, which poses challenges in applying their findings to larger energy systems and diverse scenarios. By incorporating lessons learned from prior experiences, this review aims to propel the development of SEG toward more innovative and impactful directions. It is firmly believed that positive behavior changes among individuals can be effectively encouraged by using SEG

    Churg Strauss Syndrome (CSS)is an autoimmune condition that causes inflammation of small and medium-sized blood vessels. It is a non-infectious systemic vasculitis, which affects mainly the lungs but also other tissues and organs. CSS is considered a highly variable condition in terms of its presentation and its course. The most serious complication of the vasculitic stage is congestive heart failure with reduced ejection fraction.     The impact of immediate breast reconstruction on the time to delivery of adjuvant therapy: the iBRA-2 study

    Background: Immediate breast reconstruction (IBR) is routinely offered to improve quality-of-life for women requiring mastectomy, but there are concerns that more complex surgery may delay adjuvant oncological treatments and compromise long-term outcomes. High-quality evidence is lacking. The iBRA-2 study aimed to investigate the impact of IBR on time to adjuvant therapy. Methods: Consecutive women undergoing mastectomy ± IBR for breast cancer July–December, 2016 were included. Patient demographics, operative, oncological and complication data were collected. Time from last definitive cancer surgery to first adjuvant treatment for patients undergoing mastectomy ± IBR were compared and risk factors associated with delays explored. Results: A total of 2540 patients were recruited from 76 centres; 1008 (39.7%) underwent IBR (implant-only [n = 675, 26.6%]; pedicled flaps [n = 105,4.1%] and free-flaps [n = 228, 8.9%]). Complications requiring re-admission or re-operation were significantly more common in patients undergoing IBR than those receiving mastectomy. Adjuvant chemotherapy or radiotherapy was required by 1235 (48.6%) patients. No clinically significant differences were seen in time to adjuvant therapy between patient groups but major complications irrespective of surgery received were significantly associated with treatment delays. Conclusions: IBR does not result in clinically significant delays to adjuvant therapy, but post-operative complications are associated with treatment delays. Strategies to minimise complications, including careful patient selection, are required to improve outcomes for patients

    Urban environments can be key to sustainable energy in terms of driving innovation and action. Urban areas are responsible for a significant part of energy use and associated greenhouse gas emissions. The share of greenhouse gas emissions is likely to increase as global urban populations increase. As over half of the human population will live in cities in the near future, the management of energy supply and demand in urban environments will become essential. Developments such as the transformation of the electricity grid from a centralised to a decentralised system as well as the electrification of the transportation and heating systems in buildings will transform the urban energy landscape. Efficient heating systems, sustainable energy technologies, and electric vehicles will be critical to decarbonise cities. An overview of emerging technologies and concepts in the built environment is provided in this literature review on the basis of four main areas, namely, energy demand, supply, storage, and integration aspects. The Netherlands is used as a case study for demonstrating evidence-based results and feasibility of innovative urban energy solutions, as well as supportive policies
